Creating a Form

Overview: A look at how to create new forms and use the custom form fields to get all the response types that you need.

To Create a Form

  1. Navigate to "More > Forms."1
  2. Click "Create New Form" on the top-right.
  3. Select the type of Form you want to create.
  4. Drag the fields you'd like to include from the left, over to the Form.
  5. Rename your form by clicking on "Untitled Form", and add a header image if desired.
  6. Click "Create Form" to save your form. 
1 Don't see forms listed? Be sure your account has permission to access it. To give yourself permission, see Edit Role. You may need to reach out to an Admin at your church to have them make this change for you.

Form Field Options

Descriptions

Name Field that requests for both the First and Last Name of an individual. This field links directly to the profile's Name field when linking form responses to profiles.
Phone Field that requests for a phone number. When linking form responses to profiles, this field merges with the profile's Mobile Phone field. If you'd like to collect a specific number it should be specified as such (i.e. Work Phone, Home Phone, etc.).
Email Field that requests an email address. This field links directly to the profile's Email field when linking form responses to profiles. This is where you can turn off the automatic email confirmation.
Address Field that requests an address. This field links directly to the profile's Address field when linking form responses to profiles.
Text Offers a placeholder for a text response.
Multiple Choice Gives form respondents the option to select one option specific to the field (i.e. favorite color, t-shirt size, etc.).
Checkbox Gives form respondents the option to select multiple options specific to the field (i.e. stewardship commitments, spiritual gifts).
Dropdown Gives form respondents the option to select one option specific to the field (i.e. favorite color, t-shirt size, etc.).
Essay Gives form respondents a place for more detailed text by providing space for multi-line responses.
Date A field for gathering a specific date (month/day/year).
Payment Allows you to collect a payment through your form with multiple payment options (i.e. child ticket - $5, adult ticket - $10, etc.).
File Stores up to 8mb files (i.e. photos, scanned documents, etc.).
Section Title Allows you to create distinguished section throughout your form. Can help with the clarification of different form sections and requirements.
Section Description Provides a description to each section letting users know what data is being requested as well as general information about the form. This field is HTML5 friendly, allowing you to include things such as hyperlinked text, pictures, embed videos, etc.
When using quotation marks in the form field labels or options, it will break the title.

To Print a Form

  • Click View Form and then Right-click the screen to print a blank copy of the Form. You can usually save to PDF in the Print Preview screen.
  • The easiest way to print all the form responses would be to click on "View Entries" under the form name you are wanting to print, and then instead of clicking on individual responses, click "Download as Excel" at the top of the page. 

Adding an Image to your Form:

You'll see the option to "Add a Header Image" at the top of the form editing page.  When you click on that you'll see some of the Breeze-created form headers, or you can upload your own.  The recommended size is 967px x 300px for best results.

At this time, PDF's and other files cannot be uploaded for download. See Getting a Signature on a Form.

Mobile Device Limitations: At this time Breeze is not optimized for Mobile devices using Android or iOS to create forms. This is because the "Drag & Drop Profile Field" method does not work. We are working hard to re-write Breeze coding so this might be available in the future! For now, we suggest using a computer to create Forms.

  • @Shannon you could absolutely create a form in order to easily create new database entries from, this is a great idea! While Forms cannot be embedded directly into an email, allowing your recipients to fill out and submit the from the email, you can certainly share the form you created through a link in your email, that recipients can then click on to access the form, fill it out and then submit.   • Brand new to Breeze. We're taking it for a test drive and comparing it to CCB. 

    How would we capture info like a person's ministry fit, i.e., spiritual gifts, passions, experience, interests, etc.? Would Forms be the way of getting at that? Or are forms for some other purpose?

    Welcome aboard!  That's a great question!

    It depends on who's stating the spiritual gifts.  If the person is assigning their own spiritual gift, you can set up profile fields and then set up a form with the exact field titles as the profile fields.  When someone fills a form, Breeze will give you the opportunity to update a person's profile with their answers.

    However, if you're assigning the spiritual gifts, you can just set up profile fields and track them from within a person's profile.

  • Is there a way to print the form once it is created?  I know how to print when it has been filled out by someone but don't see a way to print it blank?

    Great question, here! Thanks for taking a moment reach out. 

     

    The best way to get a blank print out of the Form is to print it once you View it! Your Form will have options View Form Entries, View Form or Share Form. You will select the option to View Form -- This will allow you to print your blank copy from this area. Just Right-click and select Print!

  • Is it possible to copy and paste fields I've already created to save some time? Can a field be copied from one form and pasted to another form? Copied and pasted within a single form? Once you make a big list in a drop down or multiple choice it would be nice to be able to copy, paste and edit instead of recreating for a second family member.

  • Is it possible to change the size of the form? For example, to be able to put some fields side by side, instead of on top of each other?  The current way makes some of my forms 5 pages.

  • Is there any way (or are you looking to add this ability) to resize the edit window for form creation?  I have fairly lengthy forms and the editing window is quite small - it would be nice to at least make it full screen, if not resizable.  Thanks!

  • @kmhagen95 I feel your pain on this one! I have several very lengthy forms at my church too and scrolling up and down so many times becomes very un-breezy! This is a great suggestion you have! Unfortunately, there is not a way to resize the edit window at this time. We currently don't take feature requests on this platform but would love for you to submit your idea here

  • Is there a way to set limits?

    We want to use Breeze for home group registration. Each home has a different number of people they are able to accommodate.

    Is it possible to show the number of available spots as well as show when a group is full?

    Also, is it possible to duplicate forms? Once I create one the others will be basically the same other than Host, Facilitator and Number of spots available.

  • @ssmithcanopy You can set a limit of Max Entries by

    1. Navigate to "More > Forms."
    2. Click to either create a new form or edit an existing form.
    3. Click the "Go to Settings" link in the bottom left.
    4. Click "show more options"

    This will limit how many forms can be accepted. You can see more about editing form settings here. However, you may run into a hiccup if you are allowing multiple people to register on one form. 

    You can easily duplicate a form by: 

    1. Navigate to "More > Forms."
    2. Click the "duplicate form icon (the two sheets of paper) to the right of the form that you'd like to duplicate
    3. Change the title of the new form
    4. Update the Host, Facilitator, and Number of Spots.
    5. Click "Create Form."

  • I'm creating a form to register kids or people for an event. Is there a way to make an "Add another child" button that generates a new section in the form for more children in the same family? Thanks for the commitment for continual improvement! We're starting to use Breeze for so many things!

    Great question! As of right now, a "fill in the blank" option isn't included within the choices, however, you could create one using the Text Box field. We know this isn't super "Breezy" but it's the most useful workaround in this case! We would love for you to request this a as Feature Request - Submitting Feature Requests

  • I'm trying to allow people to sign up to help at an event.  I have three types of jobs available.  I've created a radio button selection for the three jobs.  I only need 2 people for Job1, 2 people for Job2 and 10 people for Job3.  I see that I could create a limit on the number of form that are accepted, but that doesn't quite address my problem. 

    Can I manually delete Job1 from my form, once I see that I have two people signed up, without affecting their submitted form?

    It sounds like what you're needing to use is our Sign Up Sheets in Volunteer 2.0! Forms wouldn't give you the flexibility to do what you're trying to do. Check out this link:  Volunteer Management 2.0 - Invite Mode (BETA)

    Great question! Unfortunately, This Functionality is not available in Breeze. The sign-up sheets are sent out individually so that people cannot respond to them via their unique identifier. This is how breeze Interpret who is actually responding to the sign-up sheet. Also this functionality does not exist, I highly recommend submitting it as a feature request: Submitting Feature Requests

  • I did not see another topic like this so sorry if I missed it.  
    Is there a way to add a pdf to the form?  I am looking to add the informational pamphlet for our trip to the form in case someone has stumbled upon the form without going through our website.

    Great idea! At this time, there is not a way to upload a PDF to a Form in Breeze. You can edit the description of the form or add a section description and have a hyperlink to a place where they can download the PDF. I hope this helps!

  • Is there a way to change the size of the font being used in the forms?
    I am setting up iPad kiosks for event sigh-ups.  I created a landing page on our website and created images linking back to forms on Breeze.  However, the print is so small I am afraid our elderly will have a hard time reading it.  Increasing font size on the iPad did nothing.
